PRACTICE CHARTER STANDARDS
We are a friendly team of professionals whose aim it is to look after the health
needs of you and your family. All our staff are trained to put your needs first,
so if you are worried or unsure then do please ask and we will be pleased to help.
We are always striving to provide an even better service and we welcome your comments
which can be sent to our practice manager or to our Patients Participation Group
who liaise with us, representing your views.
COMMENTS/SUGGESTIONS
We are constantly trying to monitor and improve our services to our patients and, to this end, we conduct annual staff performance reviews and submit an annual report to NHS Croydon.
We have also established a Patient Participation Group made up of a committee of interested patients. They have set up suggestion boxes at both surgeries; comments made there are NOT seen by the doctors but discussed at regular meetings before being brought to the partners. Both comments including praise and constructive criticism are welcomed. In addition the PPG have held evening meetings on various topics. The PPG is always keen to hear from any patient who would like to become involved with the committee. Anyone interested should contact the practice manager or Pat Goodwin through reception.

PATIENTS' BEFRIENDING GROUP
We are aware how frightening it can be to go into hospital for an operation or a test. We have therefore compiled a list of our patients who have agreed to talk to other patients about procedures they’ve had done, to help allay fears and worries.
Please ask at reception or see the PPG noticeboard in the waiting room.
AUDIT
Why the
NHS collects information about you and how it is used.
What Information?
Our practice keeps records about your health and the
care you receive. This information will include:
• Your name, address,
date of birth, sex, ethnicity, nationality, first language, NHS number, next of
kin.
• A record of appointments you've had with any health professional.
• Reports on your health and the treatment and care you need.
• Results
of tests or scans you've had.
• Extra details about your health from
health professionals or others involved in your care.
Why Does
The NHS Need To Know?
All of the information that the NHS holds
about you is used to ensure that:
• Professionals caring for you
have accurate and up-to-date information to assess your health needs and decide
what care you might need in the future.
• You can be contacted if
you should need further health checks or tests.
• Full details are
available should you need another form of care, for example if you are referred
to a specialist service.
• There is a good basis for looking back
and checking on the type and quality of care you have received.
• Your concerns can be properly looked into.
By holding information about
everyone that we provide care for we can:
• Review the care we provide
to ensure it is of the highest standard.
• Plan services for the
future.
• Monitor the way public money is spent.
• Research
health and improve treatment.
• Teach and train health professionals.
This
information is normally used anonymously. When personal details are not necessary
we remove details that might identify you.
Keeping Your Information
Confidential
Everyone involved in your care has a legal duty to
keep all of your information confidential and secure. This means that information
is only shared with professionals who are involved in your care. These professionals
might include people that work for:
• GP practices
• Pharmacies, Opticians, Dentists
• Hospitals and Primary Care Trusts
• Council departments including Social Services
• NHS Direct and NHS
Walk-in Centres
• Strategic Health Authorities
• Ambulance
services
• Private hospitals and hospices
The NHS also shares
information with other organisations for essential research and auditing purposes.
This will only be done with your consent, unless the law requires information
to be passed on to improve public health.
What If I don't Want
My Information Shared?
Your consent is needed for us to share
your information and you have a legal right to refuse. We strongly recommend that
you allow us to share your information as this will make sure the best care is
available to you. We will assume your consent for us to use your information in
the manner detailed on this website unless you write to the practice manager (Mrs
Nan Nobes) to advise us otherwise.
If you do not want certain information
recorded, please talk to someone involved in your care. If you feel that you are
unable to do this, or you are not happy with the outcome then please write to
the practice manager.
Sharing Your Information Without Consent
There
are some circumstances where we are required by law to report certain information.
Only a qualified health professional can do this. These include:
• Notification of new births
• In the control of infectious diseases
• In the investigation of serious crime
• Where a court order has been
issued
Anyone who receives information from us is also under a legal duty
to keep it confidential and secure.
How You Can Get Access To Your
Health Records.
The Data Protection Act 1998 allows you to find
out what information about you is held on computer and in certain manual records.
If you want to see your health records you should make a request to the NHS organisations
where you have received treatment. You are entitled to receive a copy but a charge
will usually be made for this.
The Practice Publication Scheme
This
publication scheme is a description of the information about our general practitioners
and practice which we make publicly available. It will be reviewed at regular
intervals and we will monitor its effectiveness. The publications are all free
unless otherwise indicated and the information within each class is available
in hard copy from the practice manager.
ZERO TOLERANCE
This practice has a policy of zero tolerance. If a patient is verbally or
physically abusive the partners will consider removing them from the list. If
a patient is removed from the list at the doctor's request then:
• This
is with the agreement of all the partners.
• The practice writes to the
patient outlining the reasons for their removal and giving details of where they
can find an alternative doctor.
• The patient has the right of appeal.
• NHS Croydon are informed of the decision and the reasons for it.
